To have an effective list of revenue sources is helpful to think outside the box.
One should break ad types by ad format as well.
So one has things like

Above the scroll 468x60 and jumbo banners

Skyscrappers large and regular


below the scroll banners

Popups capped and uncapped

Popunders capped and uncapped

Interstitials, invues and the like

Search programs

Pay Per Lead Programs

Pay per sale programs

Two tier programs

Content Subscription Fees

Branded Site Merchandise

Offline Branded Sales; using a site not to sell directly but make it easier to close sales offline for complicated things like services.

and many other sources of revenue
